Physician
The main role of the hospice physician is to provide palliative care to patients with terminal illnesses. The physician ensures that the patient is as comfortable as possible and makes appropriate recommendations for further care.
Nurse
Hospice registered nurses (RNs) and licensed vocational nurses (LVNs) play a vital role in facilitating the hospice plan of care. A registered nurse case manager will be assigned to each patient to act as the main care coordinator of the individualized plan of care that they will create. The visiting RNs or LVNs will ensure that the plan of care is followed. The role of the nurses in hospice can range from symptom and medication teaching, direct patient care like wound care, catheter management, etc.
Certified Home Health Aide
Certified home health aides provide home-based care to hospice patients to aid with their activities of daily living such as feeding assistance, bathing, toileting, incontinence care, oral hygiene, and skin care, home safety procedures, and mobility and transfer assistance. They are also able to assist in providing light housekeeping / homemaker services.
Medical Social Worker
The hospice social worker assists patients and their loved ones when navigating the challenges that they experience during the end of life situations including emotional, spiritual, physical, financial, and legal obstacles. They can provide community resources that are available to patients and their families, which can help maximize their quality of life.
Chaplain
The hospice chaplain provides emotional and spiritual support to patients and their families as they face different challenges that arise with the end- of-life journey. The chaplain is also well-versed in understanding different cultural beliefs, practices, and traditions towards death and dying to help patients find comfort and meaning in their life.
Bereavement Coordinator
The bereavement coordinator facilitates the bereavement program, which consists of regular mailings, grief support groups, phone contacts, and individual visits to all members of the family for up to 13 months following the patient’s death.
Volunteer
The hospice volunteers are well-trained individuals who can provide companionship and respite to patients and their loved ones. They engage hospice patients with different activities by using their different skills and talents like crafts, music, storytelling, etc.
